Any co-payments, deductibles and co-insurances are due and payable at the time of service. Payments may be made at the registration desk anytime. Physician Billing

All physicians bill independently from the facility, and those charges are not included in your facility statement. Below is a listing of physician telephone numbers to assist you.

Pre-Registration

Patients can shorten their wait time by contacting the registration area at 270.251.4198 to pre-register for an appointment or by registering online here. Registration's hours of operation are Monday through Friday from 6:30 a.m. - 5 p.m.

Medical Records

Health Information Management (HIM) is responsible for the storage of all inpatient, observation, same day center charts, emergency department, and labor and delivery charts, including birth certificates. To request your medical records from HIM, you may complete the form found here. To contact HIM directly concerning release of information and all other medical record questions, you may call 270.251.4142 Monday through Friday from 8 a.m - 4 p.m.

Financial Assistance

Financial assistance is available to those who qualify. Jackson Purchase Medical Center has two in-house representatives available to screen patients for any assistance available. In order to be screened for assistance you may be required to provide, at a minimum, a copy of your most recent federal tax return and a recent check stub. The office of the JPMC financial counselor has moved. The counselor is accepting bill payments and financial consultations in the Emergency Room registration area. Participating Insurance Providers

Jackson Purchase Medical Center will bill your insurance company on your behalf if you have provided us with your insurance information at the time of registration. Jackson Purchase Medical Center participates in a number of networks to offer reduced savings to patients. Participation in networks is subject to change at anytime. Automated Calling System

Jackson Purchase Medical Center has an automated calling system to provide a courtesy reminder to patients about scheduled appointments for medical procedures, such as surgeries, ultrasound and MRIs. During the call, patients will be asked their first and last names and then asked to enter the last four digits of their Social Security Number on the telephone keypad. The reminder message will continue after the numbers are entered.
